In order to begin services, items below are required to be emailed or faxed for initial review
- Facesheet
- Signed MD Order
- Completed Patient Referral Form
- Plan of Care
- History & Physical (H&P)
- Recent Discharge Summary (If applicable)
- Recent Wound Photo (If applicable)
- Please send photo via Email (Include joints above and below wound)
Insurances
We accept any insurance carriers and plans. To list a few, we accept Medicare, Blue Cross, Blue Shield. If your plan is not listed above our team can verify if your specific insurance or plan is accepted upon consulting.
